Application Process
Whether you’re a clinician wanting to refer a client, you have a loved one that is interested in our services, or you are currently incarcerated and seeking a home plan, we are happy to review your application. Please find the fillable form below, or the printable PDF file at the bottom of this page. After we receive the application, we will reach out to schedule an interview. Please be sure to provide the best contact information on the application.
Application Requirements
To be determined eligible for participation in the 12-month transitional housing program, the applicant must.
• Identify as male, age 18 or older
• Be convicted of a sex offense
• Be, or willing to be, COVID vaccinated
• Comply with mental and physical health recommendations from their medical provider
• Complete a Zoom interview with staff member
• Be motivated to live a crime-free life
Program Requirements
To remain in the program, the resident must:
• Be willing and able to work and provide pay stubs
• Refrain from using or possessing drugs, alcohol, pornography, or weapons
• Meet with the case manager according to the individual case plan
• Pay $500 per month in rent (the first month is free)
• Respect all residents and staff
